Power of Attorney Medical Certification

Professional, independent medical assessment and certification for Power of Attorney documentation.

When a Power of Attorney requires medical confirmation, an experienced doctor can provide an independent assessment and, where appropriate, the relevant medical certification.

Locumdoctor4u provides professional medical certification services for individuals, families and legal professionals. Each assessment is undertaken independently and according to the individual's circumstances and the requirements of the relevant documentation.

Our role is medical, not legal.
We provide an independent clinical assessment and, where appropriate, medical certification. We do not provide legal advice or determine whether a Power of Attorney should be created.

A professional and independent service

Power of Attorney documentation may require a doctor to comment on an individual's mental capacity or other relevant medical circumstances. These assessments should be approached carefully, objectively and independently.

We aim to provide a straightforward and confidential service while maintaining the professional standards expected from medical practice.

Individuals and families

A professional medical assessment when your solicitor or legal adviser has advised that one is required.

Solicitors and legal professionals

A reliable medical referral option for clients requiring Power of Attorney assessment or certification.

What the service can involve

  • Review of the relevant circumstances and documentation
  • Private consultation with the individual concerned
  • Clinical assessment relevant to the requested certification
  • Independent medical opinion where appropriate
  • Completion of appropriate medical documentation or certification
  • Professional communication regarding the assessment where appropriate

For solicitors

Solicitors are welcome to contact us before referring a client. This can be particularly useful where a specific form, wording or other documentation requirement needs to be clarified in advance.

We recognise that legal professionals may occasionally need a medical assessment at relatively short notice and will consider appointment requests according to clinical availability.

Before arranging an appointment

Please establish with your solicitor or legal adviser exactly what medical assessment or certification is required.

If a specific form or wording has been provided, this should be made available to the doctor before or at the time of the consultation.

A medical practitioner must reach their own independent clinical opinion. We cannot guarantee that certification will be provided where the clinical assessment does not support the requested conclusion.
